Patient at the center of their health care team.Barton Health incorporates a Patient-Centered Medical Home (PCMH) approach to primary care. This is not a place, but a partnership between our patients and their primary care team.

With PCMH, you can expect:
  • Personalized care plans you help design, addressing your health concerns.
  • Medication review at each visit to help you understand and monitor your medications.
  • Coaching and advice to help you follow your care plan and meet your goals.
  • Connection to support from peers in your community who share similar health issues and experiences.

Your Health Is Important - Barton Health supports care guidelines from the United States Preventive Services Task Force (USPSTF), the American Diabetes Association (ADA), and the American Academy of Pediatrics (APP) when recommending preventive care and chronic care.

Regularly scheduled office visits allow us to better assist you in identifying and managing your health needs.


Appointments and Cancellations

We make every effort to ensure you are seen by our providers in a timely manner.

  • Patients should arrive 10 minutes prior to appointment with all paperwork completed to allow for check-in.
  • Late arrivals may not be accommodated.
  • If you need to cancel, notify your provider's office before 4:00 p.m. the day prior.
  • Your care team is available to discuss how to support you to keep your appointments.

Lab Work

We strongly encourage patients to complete any required lab work at least 3-4 days prior to your appointment. This allows your physician to discuss lab results at the time of your visit.

Patient Responsibility

Although our staff will file your medical insurance on your behalf, it is your responsibility to monitor the processing and payment of claims. If you are unable to pay your balance in full, please speak with a financial customer service representative at 530.543.5930 to learn about no-interest payment plans and financial assistance programs.

Insurance & Fees

To ensure we can correctly prepare and file your insurance claim we must obtain a copy of your driver’s license and a current insurance card(s) at each visit prior to being seen. Barton participates with most insurance plans including Medi-Cal, Medicare, and private plans. If we do not participate with your insurance carrier, payment in full is expected at each visit. If we do participate with your insurance carrier but do not have a current insurance card, you are expected to pay for your visit in full until we can verify your coverage.

Referrals

We are here to help you complete your referral request. Please ask your care team if you have questions or concerns on how to navigate the referral process.

Medication Refills

For prescription refills, please take note:

  • Approval of your refill may take up to three business days.
  • MyChart and notification from your pharmacy are the preferred methods to request prescription refills.
  • Some medications require you to be seen every three months with your primary care provider to receive a refill.
