Community Health Needs Assessment Survey

Barton Health’s commitment to the community's health is an integral part of their mission. Each year Barton is proud to invest in health programs and partner with other local organizations to identify and address the unmet needs of medically underserved populations in the south shore of Lake Tahoe.

Beginning this month, residents in the Lake Tahoe region have the opportunity to participate in a valuable community health needs survey hosted by Barton Health. Questions will address individual health choices, as well as environmental and social issues. Responses will be used to help Barton Health identify areas of opportunity and improvement for our local population, as well as foster partnerships and award health grants to help meet the outstanding health needs of our community.

Participants are randomly selected and all responses are strictly confidential. Please allow for 20-25 minutes to complete the survey. Barton thanks you in advance for your participation.

The Community Health Needs Assessment (CHNA) is conducted every three-years, the previous assessments being completed in 2018, 2015 and 2012. To view past CHNA outcomes and action plans, visit: bartonhealth.org.
