Your Case Management team provides an alternative to sending you home before you are ready. Case Management works to provide nursing care with an emphasis on rehabilitation, and works with you to prepare you for daily activities and living. This home-care setting in the hospital can assist with a smooth transition from the hospital to the home.

Criteria
You must have been hospitalized in an acute care setting for at least 3 days in the past 30 days, in addition to:
- Must be ordered by a physician.
- You must be medically stable.
- A realistic discharge plan is in place.
- Rehabilitation services need to be medically reasonable and necessary.
Insurance Coverage
Medicare and Medicaid cover Case Management for a specific length of stay. Other insurances may cover Case Management, but prior authorization is necessary.
